I authorize C2G Environmental Consultants, LLC to apply for all necessary permits for the following oil tank services:

1. Quoted price assumes the tank is free of obstructions and is accessible.

2. All work not called for in the above proposal will be an additional cost to the client. Additional costs will be agreed upon, in writing and signed by both parties, prior to additional work being completed.

3. This quotation is valid for thirty (30) days from the issue date.

4. A 50% deposit will be due upon execution of this proposal. The remaining balance will be due prior to demobilization. Deposits are not refundable for special order items. Balances will be charged to the customer's credit card and/or collected via cash, check, or money order upon delivery and installation.

5. All applicable tax will be billed in addition.

6. Work will commence on an agreed upon date within thirty (30) days of the later of either the date of deposit or the issuance of any required municipal permits. Work will be completed within three days.

1. Permits if required, are the responsibility of the property owner. However, C2G will obtain permits upon request. The cost for this service is the actual permit cost and a processing fee of $85.00. If your Building Department requires hand delivery and/or drop off this shall be your responsibility, upon request C2G can perform this service for a fee of $85.00 per hour portal to portal. Overnight fees will be billed at cost.

2. Directives of the local municipality and NYSDEC may vary this cost proposal and will be billed in addition to this proposal.

3. Encountering water, rock, void spaces and/or contamination may incur additional costs not estimated in this proposal. All additional items not in this proposal will be billed at time and material rates.

4. USTs that are deeper then thirty (30) inches below ground service may incur additional fees.

5. If the tank has more than fifty five (55) gallons of sludge/liquid a vacuum truck maybe required to facilitate removal. Additional charges for such services are $0.98 per gallon for liquid disposal and $275.00 per stop. If additional drums are required for tank bottom sludge/solids, each additional drum shall be charged at $245.00ea. If transportation and disposal of good fuel is required a flat fee of $95.00 will be charged.

6. C2G will not be responsible for delays and costs caused by others. Demurrage will be billed at $95.00 per personnel per hour.

1. C2G is not responsible for any private or public unmarked underground utilities.

2. Concrete/ asphalt restoration is to be performed by others.

3. C2G will take careful consideration of the work area; however C2G is not responsible for any damage to flooring, carpets, drywall etc.

4. C2G is not responsible for ground settling. Landscaping is not included in this cost estimate. Any replacement of shrubs, trees, sidewalk/curbing, flowers etc., is not included in this estimate. C2G will take careful consideration of the surrounding landscape but is not responsible for any damage of any landscaping caused by the scope of work.

Notice of Cancellation

1. You have the right to cancel this agreement and receive any money deposited with C2G any time before midnight of the third business day. (See attached Notice of Cancellation for details) Should you cancel the contract before work has begun, but more than three business days after the signing of this contract, C2G has the right to retain 5% of the subtotal but not more than $100.

We are required to inform you that failure to make payment in full may result in a property lien: "Any contractor, subcontractor, or material man who provides home improvement goods or services pursuant to your home improvement contract and who is not paid may have a valid legal claim against your property known as a mechanic's lien. Any mechanic's lien filed against your property may be discharged. Payment of the agreed-upon price under the home improvement contract prior to filing a mechanic's lien may invalidate such lien. The owner may contact an attorney to determine his rights to discharge a mechanic's lien".

If this agreement meets your satisfaction, please execute and return via fax or email and mail original. A 50% deposit check can be mailed or paid by credit card over the phone.

YOU THE BUYER, MAY CANCEL THIS TRANSACTION AT ANY TIME PRIOR TO MIDNIGHTOF THE THIRD BUSINESS DAY AFTER THE DATE OF THIS TRANSACTION. SEE THE ATTACHED NOTICE OF CANCELATION FORM FOR AN EXPLANATION OF THIS RIGHT.

By submitting this form, you acknowledge that you have read and understood the agreement and agree to all its terms and conditions. You furthermore acknowledge that you have the authority to enter into this agreement.
